The Hebrew Home at Riverdale by RiverSpring Living, a world-renowned long-term care organization located in Riverdale – North Bronx, serves over 18,000 people every day, both in the greater New York community and on our beautiful 32-acre overlooking the Hudson River.

RiverSpring Certified Home Health Agency provides skilled home services, including nursing, physical therapy, occupational therapy, speech therapy, and social work as well as home health aide services.

The Licensed Practical Nurse (LPN) Community Health Nurse provides skilled, direct patient care in the home setting, working under the supervision of a Registered Nurse (RN) and/or Physician, as outlined in the patient's plan of care.

Key Responsibilities

  • Promoting patient health, independence, and quality of life within the comfort of their home.
  • Observe, monitor, and report patient changes, administer treatments, and reinforce patient and caregiver education initiated by the Nurse Care Manager.
  • Provides skilled, direct patient care in the home setting, working under the supervision of a Registered Nurse (RN) and/or Physician, as outlined in the patient's plan of care.

Qualifications

  • Graduate of an approved school of nursing whose program is accredited by the National League of Nursing.
  • Current, unencumbered NYS LPN License
  • Minimum of 1-2 years of clinical nursing experience, with preference for experience in home health/hospice, acute care or sub-acute setting.
  • Ability to prioritize tasks; Able to multitask efficiently and effectively.
  • Excellent verbal and written skills with the ability to communicate information concerning a patient’s condition clearly.
  • Performs weekend on-call duties as assigned.
  • Performs other duties as assigned.
  • Knowledge of nursing and medical practices, procedures, regulations and guidelines that pertain to homecare.
  • Valid driver’s license

Compensation

  • Annual salary: $80,000 - $85,000
